There are numerous challenges pertaining to epilepsy care across Ontario, including Epilepsy Monitoring Unit (EMU) bed pressures, surgical access and community supports. We sampled the current clinical, community and operational state of Ontario epilepsy centres and community epilepsy agencies post COVID-19 pandemic. A 44-item survey was distributed to all 11 district and regional adult and paediatric Ontario epilepsy centres. Qualitative responses were collected from community epilepsy agencies. Results revealed ongoing gaps in epilepsy care across Ontario, with EMU bed pressures and labour shortages being limiting factors. A clinical network advising the Ontario Ministry of Health will improve access to epilepsy care.

OBJECTIVE
To describe the transmission dynamics of the emergence and persistence of vanA vancomycin-resistant enterococcus (VRE) in an intensive care unit (ICU) using whole-genome sequencing of patient and environmental isolates.
DESIGNRetrospective cohort study.
SETTINGICU in a tertiary referral center.
PARTICIPANTSPatients admitted to the ICU over an 11-month period.
METHODSVanA VRE isolated from patients (n=31) were sequenced using the Illumina MiSeq platform. Environmental samples from bed spaces, equipment, and waste rooms were collected. All vanA VRE-positive environmental samples (n=14) were also sequenced. Data were collected regarding patient ward and bed movements.
RESULTSThe 31 patient vanA VRE isolates were from screening (n=19), urine (n=4), bloodstream (n=3), skin/wound (n=3), and intra-abdominal (n=2) sources. The phylogeny from sequencing data confirmed several VRE clusters, with 1 group accounting for 38 of 45 isolates (84%). Within this cluster, cross-transmission was extensive and complex across the ICU. Directionality indicated that colonized patients contaminated environmental sites. Similarly, environmental sources not only led to patient colonization but also to infection. Notably, shared equipment acted as a conduit for transmission between different ICU areas. Infected patients, however, were not linked to further VRE transmission.
CONCLUSIONSGenomic sequencing confirmed a predominantly clonal outbreak of VRE with complex transmission dynamics. The environmental reservoir, particularly from shared equipment, played a key role in ongoing VRE spread. This study provides evidence to support the use of multifaceted strategies, with an emphasis on measures to reduce bacterial burden in the environment, for successful VRE control.

Six snow-pit records recovered from Siple Dome, West Antarctica, during 1994 are used to study seasonal variations in chemical (major ion and H202), isotopic (deuterium) and physical stratigraphic properties during the 1988-94 period. Comparison of δD measurements and satellite-derived brightness temperature for the Siple Dome area suggests that most seasonal SD maxima occur within ±4 weeks of each 1 January. Several other chemical species (H2O2, non-sea-salt (nss) SO42-, methanesulfonic acid and NO3-) show coeval peaks with SD, together providing an accurate method for identifying summer accumulation. Sea-salt-derived species generally peak during winter/spring, but episodic input is noted throughout some years. No reliable seasonal signal is identified in species with continental sources (nssCa2+ nss Mg2+), NH4+ or nssCl-. Visible strata such as large depth-hoar layers (>5 cm) are associated with summer accumulation and its metamorphosis, but smaller hoar layers and crusts are more difficult to interpret. A multi-parameter approach is found to provide the most accurate dating of these snow-pit records, and is used to determine annual layer thicknesses at each site Significant spatial accumulation variability exists on an annual basis, but mean accumulation in the sampled 10 km2 grid for the 1988-94 period is fairly uniform.

We present a comparison between the latest Parkes radio surveys (Filipović et al. 1995, 1996, 1997) and Hα surveys of the Magellanic Clouds (Kennicutt & Hodge 1986). We have found 180 discrete sources in common for the Large Magellanic Cloud (LMC) and 40 in the field of the Small Magellanic Cloud (SMC). Most of these sources (95%) are HII regions and supernova remnants (SNRs). A comparison of the radio and Hα flux densities shows a very good correlation and we note that many of the Magellanic Clouds SNRs are embedded in HII regions.

The introduction of mammal predators to islands often results in rapid declines in the number and range of seabirds. On Ascension Island the introduction of cats in 1815 resulted in extirpation of large seabird colonies from the main island, with relict populations of most species persisting only in cat-inaccessible locations. We describe the eradication of feral cats from this large and populated island. The campaign had to minimize risk to humans and maintain domestic animals in a state that prevented them re-establishing a feral population. Feral cat numbers declined rapidly in response to the strategic deployment of poisoning and live trapping, and cats were eradicated from the island within 2 years. During the project 38% of domestic cats were killed accidentally, which caused public consternation; we make recommendations for reducing such problems in future eradications. Since the completion of the eradication campaign cat predation of adult seabirds has ceased and five seabird species have recolonized the mainland in small but increasing numbers. Breeding success of seabirds at Ascension was low compared to that of conspecifics elsewhere, and the roles of food availability, inexperience of parent birds and black rat predation in causing this warrant further investigation. It is likely that the low breeding success will result in the rate of increase in seabird populations being slow.

The ability to repress P-element-induced gonadal dysgenesis was studied in 14 wild-type strains of D. melanogaster derived from populations in the central and eastern United States. Females from each of these strains had a high ability to repress gonadal dysgenesis in their daughters. Reciprocal hybrids produced by crossing each of the wild-type strains with an M strain demonstrated that repression ability was determined by a complex mixture of chromosomal and cytoplasmic factors. Cytoplasmic transmission of repression ability was observed in all 14 strains and chromosomal transmission was observed in 12 of them. Genomic Southern blots indicated that four of the strains possessed a particular type of P element, called KP, which has been proposed to account for the chromosomal transmission of repression ability. However, in this study several of the strains that lacked KP elements exhibited as much chromosomal transmission of repression ability as the strains that had KP elements, suggesting that other kinds of P elements may be involved.

Using pyrolysis-gas chromatography/mass spectrometry and Fourier transform infrared spectroscopy, the authors show that amber was imported into Late Bronze Age Syria and used for making the prestige artefacts found in a Royal tomb of c. 1340 BC. The objects included beads and a unique vessel in the form of a lion, likely fashioned in Syria from raw amber imported from the Baltic via the Aegean.

Children surviving treatment for malignant brain tumors commonly have problems maintaining their premorbid levels of intellectual development and academic achievement. Our group has been especially interested in the effects of treatment on normal appearing white matter (NAWM) on MRI and the influence of NAWM volumes on neurocognitive functioning. The present study assessed NAWM and attentional abilities among 37 long-term survivors of malignant brain tumors, ranging in age from 1.7 to 14.8 (Mdn = 6.5) years at diagnosis, who had been treated with cranial radiation therapy with or without chemotherapy 2.6 to 15.3 (Mdn = 5.7) years earlier. On the Conners' Continuous Performance Test, the Overall Index and 7 of the other 10 indices were significantly deficient compared to age- and gender-corrected normative values. After statistically controlling for the effects of age at diagnosis and time elapsed from treatment, 5 of the 8 indices were significantly associated with cerebral white matter volumes and/or specific regional white matter volumes of the prefrontal/frontal lobe and cingulate gyrus. No gender effects were observed. The results of the present study further support the contention that NAWM is an important substrate for treatment-induced neurocognitive problems among survivors of malignant brain tumors of childhood. Long recognized as the best potential solution to the continued scaling of the onetransistor/one-capacitor standalone dynamic random access memory (DRAM) beyond a gigabit, barium strontium titanate (BST) and other high permittivity dielectrics are fast becoming enablers to embedding large amounts of memory into a high performance logic process. System requirements such as granularity, bandwidth, fill frequency, and power pose major challenges to the use of high density standalone DRAM, leading to the current push for embedded solutions where very wide buses are possible. As a result, projected embedded memory sizes are rapidly approaching that of the standalone products, and with the high wafer cost of the combined logic plus memory process, bit cell scaling is critical. The BST memory cell, with its low thermal budget processing, very high charge storage density, and high conductivity metal electrodes has the potential to be efficiently embedded with traditional logic flows if the materials and integration challenges of the required three dimensional (3D) bit cell capacitors can be solved. BST materials properties such as dielectric relaxation, interface capacitance, and resistance degradation and their impact on capacitor scaling will be reviewed along with the electrode materials issues associated with certain 3D capacitor designs. The scaling limits of BST bit cells in the deep sub-micron regime will be discussed.

The non-spherical appearance of globular clusters was first noted by Pease and Shapley (1917) and discussed in some detail by Shapley (1930) who analyzed cluster shapes determined from star counts made by Helen Sawyer on the Franklin-Adams star charts. This classic work has provided most of the data set used in all subsequent discussions of cluster shapes. A number of studies reporting cluster shapes have appeared in the years since we began this project, the most recent of which include Geyer, Hopp and Nelles (1983), Frenk and Fall (1982), and Kadla et al. (1976, 1977).

We show that a structure built by the Inkas at the entrance to a cave at Machu Picchu was designed to admit the light of the rising sun only for a brief period before and after the December solstice. This structure is precisely constructed, unique at Machu Picchu, and may have played a part in the annual Inka ritual, Capac Raymi.

We have discovered hard and soft X-ray emission from W Puppis, the last of the four classical AM Her stars to be detected in X-rays. The orbital light curves in both soft and hard X-rays are in excellent agreement with the mean optical light curve, indicating that essentially all of the accretion luminosity originates from a very small region at the white dwarf’s magnetic pole. An X-ray dip occurs once per binary period, when the magnetic pole lies closest to our line of sight, and is probably due to absorption. The X-ray data and optical spectroscopy constrain fairly well the geometry of the system, dictating an inclina-tion angle i < 70º and a mass for the white dwarf in excess of 1.1 Mʘ. X-ray and UV observations constrain the temperature of the soft X-ray component to lie in the range 20–45 eV, while the hard X-ray component has a temperature in excess of 6 keV. The observed flux of soft X-rays is much larger than that of hard X-rays. However, when the energy band-passes of the observations are taken into account, the ratio of the soft and hard X-ray luminosities Ls/Lh = 0.5-50. The ratio is unity for Tbb = 30 eV and Tbr = 50 keV; ifc is smaller if Tbb and/or Tbr are larger and is larger in the opposite case. These results, taken together with those for AM Her using the Einstein OGS, suggest that the famous “soft X-ray problem” in the AM Her stars may have gone away.

The study of cultural stability requires a knowledge of cultural development over a reasonably long span of time. The definition of this time perspective is one of the major contributions of archaeology to the study of culture. The archaeologist therefore should be in a position to make a significant contribution to the appraisal of the stability problem itself. However, the lack of a commonly accepted anthropological definition of the concept of cultural stability imposes semantic difficulties which hinder the determination of practical limits for the stability- instability problem area. Moreover, the nature of the data available to the archaeologist conditions the kind of contribution he can make.
